If your cheeks have hollowed and your jawline has blurred, and filler seemed to fade too soon, RADIESSE is worth a look. RADIESSE® is a calcium hydroxylapatite (CaHA) injectable made by Merz. The gel creates volume right away, and while the gel is being absorbed the body forms collagen around the microspheres to support the area. Undiluted it is used for volume; diluted it is used for firmness. The doctor personally decides how to use it for each area.
About Radiesse
RADIESSE is an injectable that turns calcium hydroxylapatite (CaHA) into very small microspheres and mixes them into a gel. CaHA is a mineral also found in bones and teeth. Right after injection, the gel fills the volume. While the gel is being absorbed in the body, collagen forms around the microspheres and holds the area up. That is why it has the character of both a filler and a collagen booster. It was approved in Europe in 2004, the U.S. in 2006 and Korea in 2009 (back of the hand in 2017).

How it works
With RADIESSE, the change you see on the day differs from the change that builds from a few weeks later. The change on the day comes from the gel, and the collagen your body makes takes over after that.
The gel carrying the CaHA microspheres immediately fills the area where it is placed. Injected undiluted, volume is visible right after treatment.
While the gel is being absorbed, the body makes collagen around the microspheres. This collagen supports the area over weeks to months.
Over time the CaHA microspheres slowly break down in the body. Unlike hyaluronic acid filler, it is not a product that can be dissolved with an enzyme.

Dilution
What RADIESSE does changes with how thinly it is used. Treatment planning starts by separating the areas that need volume from the areas that need firmness.
Evidence
RADIESSE has more than 20 years of accumulated research since its European approval. From the studies in people, we present randomized comparative studies, a dilution study and a safety literature review, along with their limits.
U.S. approval trial. Randomized comparison with RADIESSE in one nasolabial fold and human-based collagen in the other
Smith S et al., Dermatol Surg 2007European multicentre randomized study. RADIESSE or one of 3 hyaluronic acid fillers in the nasolabial folds, with touch-up at 4 months and follow-up to 12 months
Moers-Carpi M et al., Dermatol Surg 2007Pilot study that injected diluted RADIESSE into the neck · décolleté and looked at collagen · elastin changes by biopsy
Yutskovskaya YA et al., J Drugs Dermatol 2017| Study | Participants · method | Results |
|---|---|---|
| Smith S et al.Dermatol Surg 2007;33 Suppl 2:S112-21 | 4 U.S. sites, 117 people with moderate or more severe nasolabial folds, randomized to RADIESSE on one side · human-based collagen on the other, blinded evaluation | Through 6 months, the RADIESSE side was better in 79%. Correction took less volume and fewer injections. Bruising · swelling were slightly more common on the RADIESSE side and mostly resolved within 14–21 days. |
| Bass LS et al.Aesthet Surg J 2010;30(2):235-8 | 102 participants from the approval trial above, followed for up to 39 months after their last injection | At 30 months or more, 40% remained 'improved' or better. No late adverse events such as nodules · granulomas · infections were reported. |
| Moers-Carpi M et al.Dermatol Surg 2007;33 Suppl 2:S144-51 | 205 people at European centres, randomized to RADIESSE vs 3 hyaluronic acid fillers, touch-up at 4 months, assessed at 4 · 8 · 12 months | At 8 months, the proportion rated 'improved' was significantly higher than with each hyaluronic acid filler. No serious adverse events. |
| Yutskovskaya YA et al.J Drugs Dermatol 2017;16(1):68-74 | 20 people with reduced firmness of the neck · décolleté, RADIESSE diluted 1:2–1:6 according to skin thickness, injected at months 0 · 4, biopsy · ultrasound · elasticity measurement | In tissue at months 4 · 7, markers of type I collagen · elastin · new blood vessels increased from baseline, and measurements of skin elasticity · dermal thickness also improved. |
| Kadouch JAJ Cosmet Dermatol 2017;16(2):152-161 | Safety literature review of 21 papers published 2004–2015, 2,779 people · 5,081 treatments | Of 173 reported adverse events (3%), 96% were nodules, and about half of the nodules (49%) occurred in areas of high movement. Most were reported as not visible and resolving without treatment. |
The first two studies are a manufacturer's approval trial and its extended follow-up, and the dilution study is a small pilot of 20 people with no control group. The human-based collagen used as a comparator is a product no longer in use. The safety literature review pools studies with differing methods, so adverse events may be under-reported. Results vary between individuals and do not guarantee results at Dr. NewCell.
Find yours
Even with the same RADIESSE, whether it is used undiluted or diluted, in which area and how much all change with your concern. Choose what bothers you most right now and we will show how Dr. NewCell uses RADIESSE for it. The final plan is decided together with the doctor, who looks at your face directly.
Undiluted RADIESSE builds support along the blurred jawline. The contour is visible right after injection, and the collagen that forms afterward holds the area up.
Undiluted · jawlineSee undiluted vs diluted →Undiluted product is injected in portions around the hollow cheeks and nasolabial folds at a depth suited to each area. Because it cannot be dissolved, the amount is kept conservative.
Undiluted · cheeks · foldsSee how it works →RADIESSE is mixed with lidocaine · saline to thin it and placed thinly over a wide area such as the lower face. There is little volume visible right away, and changes in firmness appear over time. The dilution ratio is matched to skin thickness.
Diluted · wide and thinSee undiluted vs diluted →We also look at hollowing or loss of firmness outside the face, such as the back of the hands and the neck. Whether to use it undiluted or diluted is judged separately for each area.
Firmness beyond the faceSee the plan →We first check the type, area and timing of the filler you had before. Then we choose the spots that need support and fill them with RADIESSE. For thin areas such as the lips or around the eyes, we recommend other methods.
Checking previous filler historySee who it suits →Lifting devices are used for the sagging contour, and diluted RADIESSE handles firmness over a wide area. Whether to have both, and in what order, is decided by looking at your skin.
Sagging + loss of firmnessSee combinations →This guide shows example plans by concern and is not a diagnosis. Whether to use it undiluted or diluted, in which area and how much, and which treatments to combine are decided in consultation after looking at your face and skin.
Compare
All are 'injectables that help the body make collagen', but they differ in what goes in and how the change appears. Looking at the ingredient before the name makes choosing easier.
This is a comparison of general product characteristics and does not mean one product is better than another. Even the same product progresses differently depending on dilution, amount and depth. The areas where each product can be used follow that product's approval. Which product suits you is chosen together with the doctor, who looks at your skin in consultation.
Design
With RADIESSE, what decides the result is not 'how many syringes' but 'where, undiluted or diluted, and how much'. At Dr. NewCell, the doctor personally decides the four things below in consultation.
We separate areas that need volume, such as hollow cheeks, the jawline and nasolabial folds, from areas that need firmness, such as the neck and the back of the hand.
Undiluted product goes into volume areas. Firmness areas are treated with diluted product, and the dilution ratio is set by looking at skin thickness.
Because it cannot be dissolved, the amount is kept conservative. Depth is matched to the area.
Contouring is often done in one session. When diluted product is used for firmness, sessions tend to be divided with intervals between them.
Who it fits

Process

The doctor separates areas that need volume from areas that need firmness and judges whether RADIESSE suits you. There is no separate consultation with a non-doctor coordinator.
Looking in the mirror together, we mark where it will go. For each spot we decide whether to inject it undiluted or diluted.
After anesthesia, it is injected in portions at a depth suited to each area. It takes about 20 minutes.
We finish with a cold compress and explain the aftercare precautions. The doctor who treated you continues to check your progress.
After Radiesse
Areas injected undiluted show volume right after. With diluted product there is little visible change right away.
There may be bruising, swelling and a mild foreign-body sensation, which mostly settle within a few days. A cold compress helps.
While the gel is being absorbed, collagen forms around the microspheres and the skin structure settles in firmly.
The timing of the next treatment is decided by looking at the area and its condition. How long it lasts varies between individuals.
Combination
We plan these separately: lifting devices for the sagging contour, diluted RADIESSE for firmness over a wide area.
RADIESSE fills areas that need support, such as the jawline or cheeks, and hyaluronic acid filler is used in thin areas such as the lips or around the eyes.
RADIESSE fills lost volume. Botox handles expression lines caused by movement.
